Online or phone applications are difficult due to the flood of people logging in, attempting to log in, calling in, and bugs in the system. Don’t give up. Just do it the old fashion way. Print out the form and mail it in.
The link below works. If you have trouble filling out the form varying degrees of help are available locally. If help is not available in your area just fill out the parts that you do understand and mail it in incomplete. They will call you in 1 to 2 weeks, and help you complete it.
